By signing this service Fulfillment Acknowledgment and Billing Summary, the client agrees to the following terms:

Billing Summary: The final billing summary included herein outlines all charges associated with the services provided. This includes but is not limited to labor, materials, and any additional fees as per the original agreement or amendments thereto.

Payment: Payment is due upon completion from the date of this document unless otherwise stipulated in the original Service Agreement. We accept cash, checks, or credit/debit card payments via invoice. Original pricing may have changed due to unforeseen circumstances such as significant increases in material costs, labor shortages, or unexpected site conditions.
- Late Payment: Late payments may incur interest at a rate of 5% per annum, calculated daily from the due date until full payment is received.

Legal Entity & DBA Usage: The Client acknowledges that all contracts, agreements, and legal obligations are with 'Woodhenge Builders Corp. CSLB Lic. 963558', the legal entity, which operates under the fictitious business name 'Resurx Home Services' for branding, marketing, and operational purposes. All legal rights, obligations, and liabilities under this agreement fall upon 'Woodhenge Builders Corp.'

Contractual Representation: Any reference to 'Resurx Home Services' in this context shall be understood as referring to 'Woodhenge Builders Corp.'
- Subcontractors: Woodhenge Builders Corp., d/b/a Resurx Home Services, reserves the right to engage and utilize subcontractors, independent contractors, affilliates, or agents to perform any or all of its obligations under this agreement. The use of such subcontractors will not relieve Woodhenge Builders Corp. of its responsibilities under this agreement, and Woodhenge Builders Corp. shall remain fully responsible for the work performed by any subcontractor as if it were performed by Woodhenge Builders Corp. itself. All subcontractors will be obligated to adhere to the terms and conditions of this agreement to the extent applicable to their services.
- Acceptance and Completion Acknowledgment: By accepting this document through signature, payment, or any clear affirmative action, the client agrees to the services performed by 'Resurx Home Services' and the payment terms. This acceptance also confirms that all services as per the agreement have been satisfactorily completed, barring any noted exceptions in the feedback section.
- Dispute Over Charges: Disputes regarding any charges must be communicated immediately upon receipt of this final billing summary. Failure to notify Resurx Home Services will be deemed acceptance of the charges.

Feedback and Review: The client is encouraged to provide feedback regarding the services rendered. This feedback will be used for quality assurance purposes but does not constitute an amendment to these terms unless explicitly agreed upon by both parties in writing.

Confidentiality: Both parties agree to keep the terms, pricing, and conditions of this acknowledgment and summary confidential, except where disclosure is required by law.

Warranty: Workmanship is guaranteed for 1 year from the date of completion. Warranty does not cover damage from misuse (e.g., using the installation for purposes it was not designed for) or neglect (e.g., failure to maintain as recommended), damage from external factors or normal wear and tear.

Liability: The service provider will exercise due diligence to ensure the work is completed without damage, but is not liable for pre-existing conditions or damages resulting from work performed by others.

Release of Liability: By signing this Fulfillment Acknowledgement, you agree to release Woodhenge Builders Corp., d/b/a Resurx Home Services, from liability for any pre-existing damage, conditions not related to our work, or for any issues arising from misinformation provided by you or from work done by other contractors.

Amendments: No amendment or variation of these terms shall be effective unless it is in writing and signed by both parties.
- Privacy: By agreeing to this service order, you consent to Woodhenge Builders Corp., d/b/a Resurx Home Services, collecting, using, and sharing your personal information (name, contact, payment details, project specifics) for service delivery, payment processing, project communication, marketing (if opted in), legal compliance, and security, with your rights to access, correct, or delete your data. Governing Law: Should a dispute arise, both parties agree to attempt resolution through mediation before pursuing legal action. If unresolved, this agreement shall be governed by the laws of the State of California, with legal jurisdiction in Orange County, CA.

Entire Agreement: This document, along with the original Service Order Agreement and any subsequent amendments, constitutes the entire agreement between the parties with respect to the subject matter and supersedes all prior agreements, understandings, and negotiations.
